Please arrive at least ten (10) minutes before your appointment. This will ensure we can complete all check-in and pre-examination testing before you see your eye doctor at your scheduled time.
Please note that if you are running late (and we know it happens), please call our office to inform the staff.
Please bring any current vision and/or medical insurance cards.
All minors (children under the age of 18) must be accompanied by a parent or legal guardian. Anyone else accompanying a minor must provide written permission from the minor’s parent or legal guardian in order to make decisions for the minor’s care. The required form can be downloaded here.
Please bring any eyeglasses or contact lenses you may be wearing with you for your examination.
Please have knowledge of or a copy of your systemic medications.
