Medical Insurance and Vision Plans
Smyrna Eye Group, P.C. accepts MOST VISION INSURANCE PLANS for your annual eye health examination. We are also providers for most MAJOR MEDICAL INSURANCE PLANS for your medical eye evaluations. There are differences between Vision Insurance and Major Medical Insurance.
Most Major Medical Insurance plans have a Vision Insurance rider, a separate company which provides benefits towards an eye examination, eyeglasses and contact lenses. This is similar to having a Dental Insurance rider as part of your Major Medical Insurance. These Vision Plans offer benefits which are administered by the specific Vision Insurance Plan and are usually different from your Major Medical Insurance. They will have their own exam co-pays and allowances towards eyeglasses and contact lenses.
Your Major Medical Insurance Plan would be used towards visits concerning specific eye health issues. Some examples would include:
- Cataracts
- Diabetic Retinopathy
- Glaucoma
- Red eye
- Eye pain
- Flashes & Floaters
Patients are responsible for any co-payments, deductibles and non-covered fees. These payments are due in full at the time of your office visit. Please check with your insurance carrier prior to your initial visit for specific information on your coverage(s).
When making an appointment, please have your insurance information available and bring your card(s) to your appointment.
We must have a copy of your Major Medical Insurance card on file prior to making an appointment for a medical office visit.

We do not accept Wellcare/Medicare.

These major medical plans may offer a routine vision exam as part of your coverage. The patient would be responsible for any exam co-pay or deductibles not met. We do not file any claims with our Major Medical Insurance Plans for material benefits. This would include eyeglass lenses, frames and contact lenses. The patient will pay Smyrna Eye Group usual and customary fees for these items and seek reimbursement on their own from their Major Medical Insurance Plan.
Reimbursement from your insurance, whether medical or vision, is not guaranteed for any procedures or materials.
