Please arrive 10 minutes before your appointment. If you choose not to have the Optomap Retinal Exam, we will dilate your pupils.
All minors (children 17 years or younger) must be accompanied by a parent or legal guardian.
Anyone else accompanying a minor must have written permission from a parent or legal guardian to make decisions for the child’s care. Please download a sample authorization letter here.
Smyrna Eye Group, P.C. accepts MOST VISION INSURANCE PLANS for your annual eye health examination. We are also providers for most MAJOR MEDICAL INSURANCE PLANS for your medical eye evaluations.
Patients are responsible for any co-payments, deductibles and non-covered fees. Please check with your insurance carrier prior to your initial visit for information on your coverage.
When making an appointment, please have your insurance information available and bring your card(s) to your appointment.

Any Medicaid plans will have to have eligibility prior to receiving services.
We do not accept Wellcare/Medicare.
Medicaid plans do not cover any one over the age of 21 starting as of 06/01/2013
These are Vision Insurance plans that patients may have eligibility on a routine eye exam as well as materials for glasses or contacts.

These major medical plans, may offer a routine vision exam, however we do not file any materials such as frames, lenses, add on, or contacts to any of the major medical. The patient can pay usual and customary and seek reimbursement if they would like. Reimbursement from your insurance, whether medical or vision, is not guaranteed for any procedures or materials.
We are happy to call and check on any other insurance that may not be listed above.
